There are two types of forms based on age:

The best way to return the intake forms is to scan into one document and email to SUPPORT@DOCTORDAVIDCLARK.COM
Please include any recent lab results or relevant clinical reports when returning the completed forms, as Dr. Clark will review everything at the same time. Labs and records may be sent in a separate file/email if they make the return file size too large to send in one pdf.
